It means: not valid because the date has passed (usually documents), or not fashionable.
Example 1:
I'm sorry, sir, but your passport is out-of-date. You will have to apply for another one before we can issue you a visa.
Example 2:
These acid-wash jeans are so out-of-date. The waist is so high that it nearly reaches my ribs! They must be from the 1980s!
