It means: to begin an activity with the basic, raw materials or from the beginning point. This expression is used together with many different verbs. For example

to build something from scratch
to make something from scratch
Example 1:
For the best results on this project, I suggest starting from scratch rather than trying to improve somebody else's project.
Example 2:
We built our garden shed from scratch. We bought the wood from a local lumber yard and we got everything else at a hardware store.
Example 3:
I made this cake from scratch with eggs, flour, sugar and butter. It tastes so much better than a cake from the shop!
